Client Information
Please print and complete a new client form. Valid ID must be shown at time of visit. Clients must be 18 years of age or older to present patients for any treatment or visit. If the patient has had prior vaccines or been seen at another veterinarian please call ahead to have the records sent to Baird's Animal Hospital. The records can be sent via email bairdsanimalhospital@gmail or via fax (910-739-4996).
